The patients start a fire which engulfs the building, killing all of the inmates and all but five of Vannacutt’s staff. Hearing a scream, Lance and David find Annabelle's corpse, suspended to suggest she hanged herself, but the absence of a perch immediately arouses suspicions of murder. Nora confronts Lance and tells him an unseen assailant strangled her and left her for dead. He tells her to remain out of sight so that her attacker will still think she is dead. Lance and David propose that everyone stay in their rooms and shoot anyone who enters to survive the night. Thus the innocents will have no reason to leave their rooms (and a good reason to stay inside them) and the killer must stay put or admit guilt.

The Darkness reveals that the souls that comprise it want to feed on all those "who are responsible". Upstairs, Pritchett, Eddie and Sara are trying to open one of the iron gates on the windows when they hear Price's screams from down the hall. Pritchett investigates, and The Darkness kills him while Price temporarily evades it. He figures that the only way to get out is to go to the attic and try to escape through there.

In 2007, the film was followed up with a direct-to-DVD sequel, Return to House on Haunted Hill, with Jeffrey Combs reprising his role as Vannacut and was released in both rated and unrated editions. As The Darkness prepares to assimilate Eddie, he reveals that he is adopted and not a true descendant of the original staff. The Darkness is distracted by Pritchett long enough for Eddie to escape. All three deleted scenes from the film were included on the 2000 Warner Bros. The show began as a simple yard display of tombstones and a few yard lights. Price runs ahead of them to find an escape route while Sara and Eddie stay behind. The building's security system is mysteriously tripped, locking everyone inside – a stunt which Price blames on Evelyn. Evelyn and Steven are at one another’s throats, each trying to kill the other. Jennifer, Eddie and Pritchett search the basement for the security system's control panel. While exploring the labyrinthine basement, Jennifer confesses to Eddie that her real name is Sara Wolfe, the recently fired assistant to the real Jennifer. Shortly after, Sara is nearly drowned in a tank of blood by a ghost impersonating Eddie, but the real Eddie arrives in time to save her.

Sara comes across an incoherent Price in the basement, and believing that he is Blackburn's murderer, shoots him when he approaches her. After the others return upstairs, Evelyn approaches Steven to gloat, and Steven, protected by a bullet-proof vest, attempts to kill Evelyn. The two scuffle and accidentally unleash the Darkness of the house when Price throws Evelyn through a decayed door. The Darkness is a dark, shape-shifting creature composed of the spirits in the house; this force pursues and tries to kill the remaining guests. It starts by assimilating and corroding Evelyn into itself, killing her while Price watches in horror.

Evelyn seemingly dies in front of the others, strapped to an electroshock therapy table. Price pulls a gun on the guests, demanding to know who killed his wife. Eddie knocks him out and they lock Price in the "Saturation Chamber", an archaic zoetrope device that Vannacutt used to treat schizophrenics. When the others leave, he turns the chamber on, leaving Price to be tortured by the moving images and ghostly hallucinations, resulting in a seizure. In Vannacutt's office, Sara and Eddie find a portrait of the hospital's head staff and realize that the party guests are descendants of the five survivors of the 1931 fire.

He promises to give each guest $10,000 with the stipulation that they stay the entire night in the house after the doors are locked at midnight, all the windows are barred, and there are no phones or radios to use. The guests are test pilot Lance Schroeder, newspaper columnist Ruth Bridges, psychiatrist Dr. David Trent, who specializes in hysteria, Nora Manning, who works for one of Loren's companies, and the house's owner, Watson Pritchard. All are strangers to both the Lorens and each other, their only commonality being their lust for money. Evelyn Stockard-Price (Famke Janssen), a spoiled trophy wife, is in a disintegrating marriage with Steven H. Price (Geoffrey Rush), an amusement park mogul with a wicked sense of humor, each of whom would gladly kill the other. Evelyn fancies spectacular parties, and Steven leases the house from the owner, Watson Pritchett (Chris Kattan), for Evelyn's Halloween birthday bash. Evelyn gives Steven a guest list two pages long; he shreds it to spite her and then creates one of his own.

David instead meets with Annabelle, who had faked her death using a hanging harness and sedatives. Secretly lovers, the two of them have orchestrated the various mishaps to manipulate Nora into killing Frederick. Nora, seeing Frederick enter the basement with a gun in his hand, does indeed shoot him.

Despite this, Price continues the party's theme, offering $1 million to each one who stays in the house and survives until morning, with any person not making it having his money added to the pot. After Nora, Watson, and Ruth release Lance from the secret room, Nora tells them that she shot Frederick. When they arrive in the cellar, Frederick explains that he loaded her gun with blanks, that his wife and David plotted to kill him, and that they both met their ends in the vat of acid. He says he is ready for justice to decide if he's innocent or guilty. Watson remains convinced the house is haunted, with David and Annabelle now added to its ranks of ghosts, and that he will be the next victim.

In a black-and-white post-credits scene, the spirits of the 1931 patients are seen torturing the Prices forever in the afterlife, presumably doomed to eternal damnation.

The color version was released on DVD the same year by 20th Century Fox. Extras prepared by Legend Films for the Fox DVD release included an audio commentary track by comedian Michael J. Nelson of Mystery Science Theater 3000, two versions of the trailer, and a slideshow of images from the film's original press book. The poster for the film included an illustration of a house in yet a third style, that of a fanciful four-story Romanesque structure.
